fix(proxy): resolve provider members within each group's providers

Same-name nodes from different proxy-providers were marked ambiguous and
greyed out. Resolve each group member against the group's own providers
(use / include-all, exclude-type) and bind the n-th occurrence of a name to
the n-th candidate, matching Mihomo.
